IV immunoglobulins plus methylprednisolone improves fever course in MIS-C

Children with multisystem inflammatory syndrome in children (MIS-C) treated with intravenous immunoglobulins (IVIG) plus methylprednisolone experience a more favorable fever course than children treated with IVIG alone, according to a study in JAMA.

In this retrospective cohort study of 111 children with MIS-C, 34 children were treated with IVIG and methylprednisolone, 72 were treated with IVIG alone, and 5 children did not receive either treatment. Overall, 9% of patients in the combination group and 51% of patients in the IVIG alone group did not respond to treatment. Treatments with IVIG and methylprednisolone was associated with a lower risk of treatment failure compared to IVIG alone.

Compared with IVIG alone, the combination treatment was also significantly associated with a lower risk of use of second-line therapy, hemodynamic support, acute left ventricular dysfunction occurring after initial therapy, and duration of stay in the pediatric intensive care unit.
